County Durham and Darlington NHS Foundation Trust (CDDFT) is one of the largest integrated providers of care in England.  It serves a population of over 650,000, with over 2 million patient contacts a year. 

The Trust runs services from the University Hospital of North Durham, Darlington Memorial Hospital and Bishop Auckland General Hospital.  It also runs a network of five community hospitals which provide a range of services at the heart of their local communities. Patient care is delivered by multi-disciplinary teams including primary care as well as secondary health and social care professionals.

CDDFT has a strong track record in research and created the Durham Tees Valley Research Alliance in partnership with and North Tees and Hartlepool NHS Foundation Trust and South Tees Hospitals NHS Foundation Trust.  Last year CDDFT was involved in 95 clinical trials across 25 specialties with more than 2,068 people participating.

In 2018, the Trust started providing Integrated Community Services through the County Durham Care Partnership. This formalised the strong partnership working with primary care and the local authority with care delivered through locality-based integrated ‘teams around patients’.

CDDFT was the lead Trust in establishing a pioneering regional collaborative between NHS organisations and Healthcall. This has resulted in the development and bringing to market of a number of technology-based platforms and solutions to improve patient care.
